It is indicated for long-term special care in cases of chronic conditions and compromised health, as well as rehabilitation after hospitalization. This service is available 24/7 and is supported by well-qualified physicians
Comprehensive Patient Assessment
Before we begin providing our services, our team of physicians, senior nurses, and physical therapists will assess the patient fully in order to set a care plan in place. This will include reviewing medical and personal needs and taking the time to understand what is most important to you and your family. This information is then passed to your nursing team, in order to execute the plan accordingly.
Regularly Scheduled Dr Visits
Our physicians can become your primary care physician or work with your doctor as a secondary physician during times when your loved one may not feel well enough to visit the doctor’s office. The physician will also monitor the patient’s health, and take note of any changes in their condition. Our physicians will also coordinate with emergency rooms and doctors from other specialties.
Tube Feeding Nutrition Therapy
Certain illnesses and surgical procedures can make feeding difficult. Our nurses are well qualified to handle special feeding requirements. Nutritional therapy can provide the essential dietary nutrition required for a healthy status.
Our home dialysis nurses provide 24/7 coverage for our home patients. They are experienced in peritoneal dialysis and facilitate home hemodialysis.
Our speech therapists work as a team with your doctor, audiologist and psychologist to evaluate and diagnose a patient’s problem with speech and communication. Once the cause is determined, a customized treatment plan is formulated keeping in mind the functional, environmental and social needs at home. Speech therapy also involves correction of swallowing abnormalities to improve nutritional intake and prevent the entry of food into the lungs.
Rehabilitation (Physical and Occupational Therapy)
Our rehabilitation services use modalities such as heat, ice, and techniques such as manipulation and massage. Interventions may include therapeutic exercise, functional training, assistive and adaptive devices, equipment, and physical agents to help relieve pain, improve balance, flexibility, strength and coordination, and ensure safe and independent living.
Prevention of Pressure Ulcers and Wound Care
Bedridden patients are at a high risk of developing bedsores or pressure ulcers. Bedsores or pressure ulcers are areas of skin breakdown that occur due to prolonged pressure applied over bony areas such as the hips, ankles and heels as a result of being confined to a bed or wheelchair for a long time. Our nurses will apply safety measures to prevent the occurrence of bedsores, and facilitate healing of existing ones.

With state of the art technology, testing can now be done in the comfort of your loved one’s home, instead of sending them to the hospital. Through the use of portable equipment, tests can be conducted by our trained professionals in the comfort of your home.
